Understanding Metabolic Health

What exactly is "metabolic health," and how is it different from just being at a healthy weight?

Metabolic health refers to how well your body processes and uses energy at a fundamental level. It encompasses five key markers: blood sugar, blood pressure, cholesterol (HDL and triglycerides), waist circumference, and body composition. Poor metabolic health is the precursor to advanced diseases including cardiovascular disease, heart attacks, strokes, kidney disease, and numerous types of cancer.

You can be at a "normal" weight and still have poor metabolic health — and conversely, some people carry extra weight but have excellent metabolic function. True metabolic health means all of these systems are working in harmony, keeping your energy stable, your risk of chronic disease low, and your body functioning at its best.

How do I know if my metabolic health is poor? What are the warning signs?
Many metabolic conditions develop silently — you can have elevated blood sugar or high blood pressure for years without feeling any different. That said, common warning signs include persistent fatigue, difficulty losing weight despite effort, brain fog, poor sleep quality, frequent hunger or energy crashes, and increasing waist circumference. The only way to know for certain is through targeted lab testing and a comprehensive assessment, which is exactly what we offer at Promotive Metabolic Health.
The site says only 12% of Americans are metabolically healthy — does that mean I'm probably not?
Statistically speaking, the odds are not in your favor — but that's actually the point. Research from the University of North Carolina found that only about 1 in 8 American adults meets all the criteria for optimal metabolic health. This doesn't mean you're sick, but it does mean that most of us have room for meaningful improvement. Even those who do meet all the criteria for good metabolic health can learn how to maintain that good health. The good news is that metabolic health is highly responsive to targeted lifestyle changes, and even modest improvements can significantly reduce your long-term disease risk.

What To Expect

Can Promotive Metabolic Health replace my primary physician?
No, we do not provide the full spectrum of primary care services. Our program is designed to improve your long-term health, prevent the development of many chronic conditions, and extend your years of active, vibrant living. However, we do not offer many services that a primary care physician does, including immunizations, care for acute illnesses or injuries, prescription medications for other conditions, and many other services. We strongly recommend you maintain your current health insurance and primary physician.
What actually happens during the free 15-minute consultation?
The free consultation is a relaxed, no-pressure conversation — not a sales pitch. We'll spend time understanding your health history, current concerns, and goals. You'll learn which of our programs might be the best fit for you, what the assessment process looks like, and what kinds of results our patients typically experience. There's no obligation to enroll, and you'll walk away with a clearer picture of your options regardless of what you decide.
What is a DEXA scan, and why is it useful for metabolic health?
DEXA (Dual-Energy X-ray Absorptiometry) is a low-radiation imaging test that provides a precise breakdown of your body composition — specifically how much of your body is lean muscle, fat, and bone. Unlike a standard scale or a BMI calculation, DEXA can pinpoint where fat is stored in your body, which matters enormously for metabolic risk. Visceral fat (fat stored around your organs) is far more metabolically dangerous than subcutaneous fat, and DEXA is one of the best tools available for measuring it accurately. It also shows how much muscle you have, which directly correlates with your metabolic health and long-term health preservation. Bone density is another critical factor to preserve for your long-term health and physical function.
What is a VO2Max test, and do I need to be an athlete for it to be relevant?

VO2Max measures your body's maximum capacity to use oxygen during exercise — essentially, your cardiovascular fitness ceiling. You absolutely do not need to be an athlete for this to be meaningful. Research consistently shows that VO2Max is one of the strongest predictors of long-term health and longevity across all fitness levels. Even modest improvements in VO2Max are associated with significantly lower risks of cardiovascular disease, metabolic dysfunction, and all-cause mortality.

Beyond the single VO2Max number, our testing also reveals how well your body burns both fat and carbohydrates. This is a critical function that underlies your metabolic health. We use your VO2Max result to design an exercise protocol calibrated precisely to your current health and goals.

What lab tests are included, and do I need a referral from my primary care doctor?

Our metabolic profile includes a comprehensive set of blood markers that go well beyond a standard annual checkup — including fasting glucose, insulin, HbA1c, a full lipid panel with advanced fractionation, inflammatory markers, and more. This gives us a clear picture of the current state of your metabolic health as well as the health of individual organ systems.

No referral is required. As a direct-care clinic, we work independently of the traditional insurance-driven healthcare system, which means you can come to us directly and we can coordinate with your primary care provider if you'd like us to.

Programs & Pricing

Is the Snapshot Assessment a prerequisite before joining the Core Membership?
The Snapshot Assessment and Core Membership are designed as two distinct entry points depending on your goals and readiness. The Snapshot Assessment is ideal if you want a thorough biological baseline before committing to a longer program. The Core Membership includes all of the same initial assessments, plus ongoing communication and support, additional testing (may require additional fees), quarterly check-ins, and repeat testing at the six-month mark. You can start with either — we'll help you decide which is the right fit during your free consultation.
Is insurance accepted, or is this a cash-pay program?
Promotive Metabolic Health operates as a direct-pay clinic, meaning we do not bill insurance. This model allows us to spend significantly more time with each patient, offer testing and services that insurers typically don't cover, and focus entirely on your long-term health rather than what a third-party payer will approve. Many patients find that the depth and personalization of care they receive here is well worth the investment, and some are able to use HSA or FSA funds to offset the cost. We're happy to discuss the details during your consultation.
What does "ongoing support" actually look like — calls, visits, an app?
Our Core Membership includes regular communication structured to fit your life. After your initial comprehensive assessment, you'll have structured quarterly check-ins to review your progress, adjust your plan, and address any new questions. Between check-ins, you have access to ongoing support from our clinical team. Everything is built around accountability and real-world practicality — we're not handing you a PDF and wishing you luck. The format of communication can be tailored to what works best for you.

Eligibility & Access

Is this only for people with a diagnosed condition, or can healthy people benefit too?
Anyone can benefit from our in-depth health analysis. If you have an existing condition like prediabetes, hypertension, or metabolic syndrome, our programs are designed to address the root causes and help reverse dysfunction — not just manage symptoms. But some of our most motivated patients come to us in apparently good health, driven by a desire to optimize performance, extend healthspan, and get ahead of risks before they become problems. Whether your goal is restoration or optimization, we can identify risks to your health and offer the best evidence-supported interventions.
Is this available only to California residents, or can out-of-state patients participate?
Our services are currently only available to California residents. We will add additional locations as our practice grows.

Lifestyle & Approach

How is this different from seeing my primary care doctor, hiring a nutritionist, or using an app?
Primary care is essential, but it's largely reactive — you're seen when something is wrong, and visits are short. Nutritionists can be excellent for dietary guidance but typically lack the medical testing and clinical oversight we provide. Apps offer useful data but no physician-led interpretation or personalized clinical plan. At Promotive Metabolic Health, you get a physician-directed, evidence-based program that integrates advanced diagnostics, body composition analysis, cardiovascular fitness testing, nutrition, exercise, and longitudinal care into a single cohesive plan built around your individual health and goals. It's a fundamentally different level of depth and accountability.
